Fleming facilitated access to another supplier, Jasveen Sangha, dubbed the “Ketamine Queen,” who was known for catering to high-profile clients. On October 12, Plasencia administered a shot that caused Perry to freeze up and experience a spike in blood pressure, prompting the doctor to suggest caution in future administrations.

By mid-October, Perry sampled Sangha’s ketamine, prompting a substantial order of 25 vials for $5,500. Concurrently, Perry received a legal ketamine treatment from his physician, who noted improvements in his depression and reduced the frequency of treatments, despite another close acquaintance believing Perry had remained sober.

In the days leading to October 28, Iwamasa sourced another batch of 25 vials from Sangha, referring to her supplier as “Master Chef.” Flemming facilitated these exchanges, ensuring a steady supply. Over October 25-27, Iwamasa administered at least six shots of ketamine daily to Perry, who was allegedly in high spirits according to those who spoke to him.

The final sequence of events began early on October 28. Around 8:30 a.m., Iwamasa injected Perry, later reporting that Perry played pickleball that morning, although the veracity of such claims varied. By 12:45 p.m., Perry received a second injection and settled in to watch a movie. Shortly before 1:30 p.m., Perry requested a “big one” from Iwamasa, receiving his third and final dose while lounging near his jacuzzi. When Iwamasa returned from errands around 4 p.m., he discovered Perry unresponsive, face down in the jacuzzi. Despite immediate efforts and the arrival of paramedics, Perry was declared dead. The cause of death, according to the coroner’s report, was primarily due to ketamine, with drowning listed as a secondary factor.

The aftermath sees varying degrees of accountability among those involved. Iwamasa has pleaded guilty to conspiracy to distribute ketamine, while Fleming has admitted to distributing the drug resulting in death. Both are cooperating with the authorities. Chavez has also agreed to plead guilty to conspiracy charges. However, Plasencia and Sangha, pivotal figures in the ordeal, have pleaded not guilty to multiple felony counts, with Plasencia’s attorney asserting his actions were in Perry’s best medical interest.
